Submit Rx
Your sleep doctor or pulmonologist typically prescribes CPAP therapy; however, any of the following providers can also provide your CPAP prescription:
- D.O. (Doctor of Osteopathy)
- M.D. (Medical Doctor)
- Psychiatrist
- P.A. (Physicians Assistant)
- N.P. (Nurse Practitioners)
- Dentist
Your Rx must include:
- Provider's information
- Signature
- Date
- NPI Number
Upon receipt of your prescription, our staff will contact your provider to confirm the settings, equipment, supplies and dates of service. Once we have verified your Rx, you will receive a confirmation email from support@cpapnetwork.com, stating that all of your prescription criteria has been validated, clearing the way for your order to be be fulfilled.
